Previous years' forms will NOT be accepted.
Only 43 Players and 14 Coaches should fit on 1 page.

Come prepared with 20 copies of the Official Roster for the A-team and 20 copies of the Official Roster for the B/C-team -- all SIGNED by the HEAD COACH.
--List in alphabetic order by player LAST NAME.
Accompanying the Official Roster must be a Registration Form with birth certificate copy stapled for EACH PLAYER.
A list of any Registration Forms WITHOUT attached birth certificate copies must be supplied with the Official Roster.
